How To Enroll

Visit the CalOptima Health PACE Center in Garden Grove. It’s the first of its kind in Orange County. It combines medical care, home care, rehabilitation, activities and transportation.

We are open Monday through Friday from 8 a.m. to 4:30 p.m. We have staff who speak your language. TDD/TTY users can call toll-free at 1-714-468-1063.

To qualify for PACE

You must:

  • Be at least 55 years old
  • Live in Orange County
  • Meet the nursing facility level of care requirements as determined by the State of California
  • Be able to live safely at home or in a community setting with proper support
