In our Health Education Library, you will find self-care guides and brochures about quitting smoking, asthma, diabetes, nutrition, exercise and more!

Medicare Diabetes Prevention Program For OneCare Connect

Starting April 1,2018, all health plans will cover Medicare Diabetes Prevention Program (MDPP) services for people who are on Medicare. MDPP is a health behavior change program created to help participants learn about living a healthy lifestyle to prevent diabetes. This program teaches practical ways to make healthy diet changes, increase physical activity and address barriers to help maintain weight loss goals. The program offers interactive sessions in a classroom-style setting.

Please call our Health Management department at 1-714-246-8895 or CalOptima OneCare Connect Customer Service toll-free at 1-855-705-8823 if you have questions or to find out if you are eligible for this program. We have staff who speak your language. TDD/TTY users can call toll-free at 1-800-735-2929. The Health Management department is here to help you Monday through Friday from 8 a.m. to 5 p.m.

OneCare Connect Cal MediConnect Plan (Medicare-Medicaid Plan) is a health plan that contracts with both Medicare and Medi-Cal to provide benefits of both programs to enrollees. Limitations, co-pays, and restrictions may apply. For more information, call OneCare Connect Customer Service or read the OneCare Connect Member Handbook. Benefits and/or copayments may change on January 1 of each year. Copays for prescription drugs may vary based on the level of Extra Help you receive. Please contact the plan for more details. Other Pharmacies/Physicians/Providers are available in our network. The List of Covered Drugs and/or pharmacy and provider networks may change throughout the year. We will send you a notice before we make a change that affects you.
