OneCare Connect Changed to OneCare

On January 1, 2023, the OneCare Connect plan changed to OneCare (HMO D-SNP), a Medicare Medi-Cal Plan. If you were in OneCare Connect, you can still find member information online such as:

Member documents like plan and pharmacy materials
Member rights information like grievances, appeals and State Fair Hearings

OneCare Connect information will be available online until December 31, 2024.
