Street Medicine Program

CalOptima Health’s Street Medicine Program delivers health care directly to members experiencing homelessness. With mobile medical vans and outreach teams, the program ensures access to essential services, including medical care, mental health support, case management and housing resources.

Health Care on the Move

Breaking barriers to care, the Street Medicine Program takes a comprehensive approach to addressing the diverse health needs of the unhoused. Mobile clinical teams provide medical care where members are, and peer navigators coordinate support services. Ultimately, the program aims to connect members with permanent housing to foster stability and long-term wellness. By meeting people in the community, our Street Medicine Program promotes resilience and transforms lives.

Partner Cities

CalOptima Health collaborates with cities across Orange County to deliver life-changing care to members experiencing homelessness. Through these partnerships, the Street Medicine Program delivers an innovative service model that addresses immediate health needs while helping members achieve long-term stability and housing.

Garden Grove

CalOptima Health’s Street Medicine Program launched in Garden Grove in April 2023 in partnership with Healthcare in Action. With an initial $4 million investment, the program provides primary care, behavioral health services and housing resources using mobile medical vans. Garden Grove was the first city in Orange County to embrace this transformative approach, improving the lives of hundreds of residents.

Costa Mesa

In August 2024, Costa Mesa joined the Street Medicine Program in collaboration with Celebrating Life Community Health Center. Covering 16 square miles, the program uses a customized mobile exam room to deliver care directly in the community. With approximately 300 people experiencing homelessness in Costa Mesa, this initiative provides critical health services and housing support to those who need it most.

Anaheim

Anaheim became part of the Street Medicine Program in September 2024 through a partnership with Healthcare in Action. This program complements the city’s robust efforts to address homelessness, including shelters, mental health services and innovative court programs. Using a mobile medical team, the program delivers care directly to unhoused people, helping them move toward wellness and housing stability.
