At PACE, our Interdisciplinary Team (IDT) of professionals plan, coordinate and deliver you personalized health care. They include:

  • Specialists
  • Nurses
  • Pharmacists
  • Social workers
  • Dieticians
  • Physical therapists
  • Occupational therapists
  • Home care staff
  • Activity recreational staff
  • Transportation staff

Care plan

The IDT works with you and your family to create an individual care plan, that is managed on a daily basis. By seeing you often at PACE, the IDT can see if your health changes, and modify your care plan.

Our program is designed to be small and personal, so that we can get to know you and your preferences. We work together to keep you in your home and as independent as possible.
