How to Join CalOptima Health PACE Program

Our Program of All-Inclusive Care for the Elderly (PACE) is a long-term comprehensive health care program that helps older adults to stay as independent as possible.

Get Started On Your Path to PACE

You’ll get PACE services at your home or at our PACE Center. Follow these steps to enroll:

Step 1

Speak With Us

Contact us to see if PACE is the right fit for you. We’ll learn about your health care needs and explore how our PACE program can help you, including Alternative Care Settings (ACS) and community-based doctors.

Step 2

Check Eligibility

We’ll confirm if you meet the requirements to enroll in our PACE program.
Step 3

Plan a Home Visit

One of our team members will visit you at your home to further check your eligibility for our PACE program.

Seek State Approval

We’ll submit your application to the California Department of Health Care Services (DHCS) for review and approval.

Step 5

Complete Your Application

Once approved by DHCS, we’ll help you finish the enrollment application.

Step 6

Start Getting PACE Services

Congratulations! You’re now a PACE participant and will start getting PACE services on the first of the following month.

Understand Our PACE Program Requirements

To qualify for our PACE program, you must: 

  • Be age 55 or older
  • Live in our service area
  • Meet the state’s requirements for nursing home care
  • Be able to live safely at home or in the community with our PACE program services

How much you might need to pay for our PACE program can vary based on the type of insurance coverage you have. See the chart below to understand the differences.
