Long-Term Services and Supports

CalOptima Health has offered a long-term care (LTC) benefit for members since 1998. This history gives us a great deal of experience in caring for members entering and residing in LTC facilities. Working with our provider partners, we developed a program with a member-centered approach.

Long-Term Services and Supports Facilities

CalOptima Health members deserve quality long-term care, which is why we work with nearly 400 local long-term care facilities. Each facility must go through a rigorous selection and contracting process to ensure CalOptima Health members receive consistent, quality care in a comfortable and secure environment. Multipurpose Senior Services Program (MSSP)

MSSP provides social and health care management services to delay older adults from having to go to nursing homes. MSSP care managers help CalOptima Health members get needed services from community resources while working with their families and support systems. MSSP lowers costs and allows members to stay in their homes and enjoy a higher quality of life.

MSSP is funded by the California Department of Aging and is administered by CalOptima Health.

Care management to help elderly adults remain at home

Eligibility guidelines

To get these services, CalOptima Health members must:

  • Be 65 years of age or older
  • Live in Orange County
  • Have a need for nursing home care
  • Be on Medi-Cal with no share of cost

Care management services

Registered nurses (RNs) and social workers provide care management services to MSSP members and their families. Some services include:

  • Making a full review of the member’s and family’s needs
  • Creating a care plan with the member to meet his or her needs through family and community resources
  • Purchasing services or items that are not offered through family and community resources
  • Linking the member to services that meet the care plan goals
  • Reviewing the member’s needs regularly to track the progress of services
  • Helping the member keep services and find new ones

Facilitating Member Needs

MSSP care managers coordinate a wide range of services based on the member’s needs:

  • Community-Based Adult Services (formerly Adult Day Health Care)
  • Medical supplies such as walkers, canes, grab bars, wheelchairs, hospital beds, bath chairs, etc.
  • Non-medical supplies such as medical alert systems, ramps, heaters, fans, etc.
  • Personal care, home chore services and caregiver relief
  • Transportation
  • Minor housing repairs
  • Counseling for mental illness and medical issues

Community-Based Adult Services (CBAS)

Community-Based Adult Services offers assistance and services to low-income, elderly or disabled individuals. Participating seniors and disabled individuals receive services at a local community adult day care center, allowing participants to continue living at home, with the intention to minimize the need for placement in nursing homes.

CBAS

Eligibility guidelines

In addition to being a CalOptima Health member, to be eligible for CBAS, participants must:

  • Be 18 years old or older
  • Have a physical, behavioral or memory problem
  • Need to live in a long-term care facility if you do not get CBAS services

Benefits of program

CBAS participants receive the following benefits:

  • Nursing services — to observe and monitor your health and medicines, and supervise your personal care
  • Personal care/social services — to help you with hygiene and other daily tasks
  • Physical therapy, speech therapy and occupational therapy
  • Meal service — to serve you at least one meal per day at the center
  • Transportation — to and from the CBAS center, as necessary

CBAS Frequently Asked Questions

  • What is the Community-Based Adult Services (CBAS) program?

    CBAS gives you important health care services at a local community center. This means you can stay where you live, and you don’t have to move to a nursing home.
  • Can I continue to see my Medicare provider if I receive CBAS from CalOptima Health?

    Yes. If you have both Medicare and Medi-Cal, you can continue to see your Medicare providers while receiving CBAS from CalOptima Health.
  • Do I have to pay for CBAS?

    No, CBAS services are covered by CalOptima Health.
  • How do I receive CBAS?

    We want to make sure you get services you are eligible for and that we have all of the information about your health. The steps below explain how we decide if you are eligible for CBAS:

    1. Talk to your doctor or call CalOptima Health’s Customer Service department.
    2. CalOptima Health will schedule an in-person meeting with you to see if you qualify for CBAS.
    3. If you qualify, CalOptima Health will work with you to choose a CBAS center that best meets your needs.
  • What if I am not eligible for CBAS?

    If you are not eligible for CBAS, our case management staff can work with you to find services in the community to meet your health care needs.
  • What if I have questions or problems?
