Care management to help elderly adults remain at home

Multipurpose Senior Services Program (MSSP) provides social and health care management services to delay older adults from having to go to nursing homes. MSSP care managers help CalOptima members get needed services from community resources while working with their families and support systems. MSSP lowers costs and allows members to stay in their homes and enjoy a higher quality of life.

To get these services, CalOptima members must:

  • Be 65 years of age or older
  • Live in Orange County
  • Have a need for nursing home care
  • Be on Medi-Cal with no share of cost

Care management services

Registered nurses (RNs) and social workers provide care management services to MSSP members and their families. Some services include:

  • Making a full review of the member’s and family’s needs
  • Creating a care plan with the member to meet his or her needs through family and community resources
  • Purchasing services or items that are not offered through family and community resources
  • Linking the member to services that meet the care plan goals
  • Reviewing the member’s needs regularly to track the progress of services
  • Helping the member keep services and find new ones

MSSP care managers coordinate a wide range of services based on the member’s needs:

  • Community-Based Adult Services (formerly Adult Day Health Care)
  • Medical supplies such as walkers, canes, grab bars, wheelchairs, hospital beds, bath chairs, etc.
  • Non-medical supplies such as medical alert systems, ramps, heaters, fans, etc.
  • Personal care, home chore services and caregiver relief
  • Transportation
  • Minor housing repairs
  • Counseling for mental illness and medical issues
