Welcome To CalOptima PACE!

PACE (Program of All-Inclusive Care for the Elderly) is a community-based Medicare and Medi-Cal program that provides coordinated and integrated health care services to frail elders to help them continue living independently in the community.

PACE provides all the acute and long-term care services covered by Medicare and Medi-Cal. The services are arranged just for you, based on your needs as indicated by our Interdisciplinary Team.

Services offered by CalOptima PACE include:

  • Routine medical care, including specialist care

  • Prescribed drugs and lab tests

  • Personal care for things like bathing, dressing and light chores at home and in our Day Center

  • Recreation and social activities, and nutritious meals

  • Rides to health-related appointments, and to and from our Day Center

  • Social services

  • Hospital care and emergency services*

Depending on your medical need, additional services may include:

  • Dental, vision, podiatry and hearing services (dentures, glasses, foot care and hearing aids)

  • Physical, occupational and speech therapies

*Participants must receive all needed services, other than emergency care, from CalOptima PACE providers and will be personally responsible for any unauthorized or out-of-network services.

To be a PACE participant, you must:

  • Be at least 55 years old

  • Live in our service area

  • Be determined eligible for nursing facility services by the State of California

  • Be able to live safely at home or in a community setting with proper support

We are happy to give you a tour of our PACE center, which will provide you with more information about our services and benefits. If you are interested in becoming a participant after this tour, we will arrange a convenient time to meet with you again to assess your needs, answer any questions and explain our services in full detail.

Summary Of Benefits And Coverage Table
CalOptima PACE offers a variety of program benefits, covered by Medicare and Medi-Cal. Your specific Interdisciplinary Team will work with you and your family to determine which benefits are best for you. This table is intended to help you compare coverage benefits and is a summary only. There are no co-payments for PACE services.

The Grievance Process

All of us at CalOptima PACE share responsibility for your care and your satisfaction with the services you receive. Our grievance procedures are designed to enable you and/or your representative to express any concerns or dissatisfaction you have so that we can address them in a timely and efficient manner. You also have the right to appeal any decision about our failure to approve, furnish, arrange for or continue what you believe are covered services, or to pay for services that you believe we are required to pay.
