Your Health Care

Having a provider you trust makes you feel good about the health care services you get. That’s why OneCare Connect Cal MediConnect Plan (Medicare-Medicaid Plan) gives you choice of more than 3,800 doctors and specialists.

Plus, OneCare Connect makes your good health a team effort. Your doctors, home and community services providers work with your Personal Care Coordinator and you to create a care plan that’s right for your health care needs.

Your Providers

Your primary care provider (PCP) helps you with your medical needs. For some services, you may need a referral from your PCP. The OneCare Connect Provider and Pharmacy Directory gives you a list of the PCPs, specialists, hospitals, pharmacies, skilled nursing facilities and outpatient mental health providers for you to choose. All providers have received Cultural Competency training.

Your Health Network

A health network is a group of doctors, nurses, hospitals, and others who provide health care services. It is important to choose a health network that meets your health care needs.

Your Pharmacy

The right medications can help you feel better and better manage your health conditions. You can choose from more than 400 pharmacies to get your prescriptions filled.

OneCare Connect Cal MediConnect Plan (Medicare-Medicaid Plan) is a health plan that contracts with both Medicare and Medi-Cal to provide benefits of both programs to enrollees. Limitations, co-pays, and restrictions may apply. For more information, call OneCare Connect Customer Service or read the OneCare Connect Member Handbook. Benefits and/or copayments may change on January 1 of each year. Copays for prescription drugs may vary based on the level of Extra Help you receive. Please contact the plan for more details. Other Pharmacies/Physicians/Providers are available in our network. The List of Covered Drugs and/or pharmacy and provider networks may change throughout the year. We will send you a notice before we make a change that affects you.
