Welcome to OneCare Connect Cal MediConnect Plan (Medicare-Medicaid Plan)!

OneCare Connect is a new health plan that combines your Medicare and Medi-Cal benefits in one plan. At no extra cost, you also get vision care and transportation to medical appointments.

A Personal Care Coordinator works with you and your doctors to create a health care plan that fits your needs. Plus, you can get support and services you need at any time. It makes getting quality health care easier!

To be a member of OneCare Connect, you must:

  • Live in Orange County

  • Have both Medicare Parts A and B and Medi-Cal

  • Be 21 years and older

  • Not receive services from a regional center, and not be enrolled in certain waiver programs

AtOneCare Connect, we believe in the importance of providing services that are sensitive to the needs of the community. We want to make sure you have access to the best health care possible. OneCare Connect respects you, your culture and your beliefs. We want you to feel comfortable talking with us and your health care providers.

To Make It Easier For You, OneCare Connect Provides:

  • Bilingual staff to help you in your language

  • Interpreter services, including American Sign Language, at no cost to you for all of your health care needs. You do not need to ask friends or family members to interpret for you. You can get interpreter services 24 hours a day, 7 days a week for:
    Medical services: doctor visits, after-hours services, urgent care services, pharmacy services and health education classes

    Non-medical services: customer service, member complaints and member orientation meetings

  • Health education and enrollment materials printed in several languages at no cost to you

  • Upon request, materials in other formats, such as Braille, audio or large print

All you need to do is call your health network or OneCare Connect. For scheduled appointments, make sure to ask for an interpreter at least 5 working days before your appointment.

OneCare Connect Cal MediConnect Plan (Medicare-Medicaid Plan) is a health plan that contracts with both Medicare and Medi-Cal to provide benefits of both programs to enrollees. Limitations, co-pays, and restrictions may apply. For more information, call OneCare Connect Customer Service or read the OneCare Connect Member Handbook. Benefits and/or copayments may change on January 1 of each year. Copays for prescription drugs may vary based on the level of Extra Help you receive. Please contact the plan for more details. Other Pharmacies/Physicians/Providers are available in our network. The List of Covered Drugs and/or pharmacy and provider networks may change throughout the year. We will send you a notice before we make a change that affects you.
