Covered Benefits (2017)

With OneCare (HMO SNP), you get total care. All your benefits are in one plan.

Our generous health care coverage includes:

  • Prescription drug benefits

  • Doctor or specialist visits - always at $0 cost

  • $0 cost medical transportation - 60 one-way taxi rides per year

  • Acupuncture - $0 for up to 24 visit(s) every year

  • Enhanced dental benefits

Vision benefits include:

  • Vision exam every year - $0 cost

  • Every two years, up to $150 for eyeglasses (frames and lenses) or up to $50 for contact lenses

A complete list of benefits can be found in the Evidence of Coverage. This explains your rights, benefits and responsibilities as a OneCare member and is in effect from January 1, 2017, through December 31, 2017. Please read it and keep it in a place that is easy to find.

  • To learn more about OneCare, you can sign up to attend one of our Member Orientation meetings. At this meeting, you will learn:

    • How to get medical services
    • How to get an authorization for your medication
    • How to use community resources
    • About your member rights and responsibilities
