Members new to CalOptima need to choose a health network and Primary Care Provider (PCP). When you become a CalOptima member, you receive a new member packet with a Health Network Selection Form and Health Network and Provider Directory.
The Health Network and Provider Directory is a list of health networks, PCPs, and hospitals.
You may choose any of the doctors listed in the Health Network and Provider Directory as your PCP. We will do our best to give you the PCP that you choose. If the PCP you chose is not open to new members, your health network will choose one for you.
You may change PCPs every 30 days. If you change doctors, please fill out the Medical Release Form and return this form to your new doctor’s office.
You and your eligible family members may ask to change health networks every 30 days. To do this, you need to complete a Health Network Selection Form. You may call CalOptima’s Customer Service Department to ask for a form. CalOptima must receive your form by the 10th of the month for your health network change to be effective the 1st of the following month.
