Change Your PCP

You can change your primary care provider (PCP) every 30 days if the PCP you choose is contracted with your health network. If you want to change your PCP, search for a provider using the online provider search tool. When you have found the provider you want, click the “Make this My PCP” button on that provider’s search details page, then follow the instructions to complete the request form.
